Daniel Ariely has posted this picture of a German 1938 ballot paper which guides German voters (if guidance were needed) in their response to the question: Do you agree with the reunification of Austria with the German Reich that was enacted on 13 March 1938 and do you vote for the party of our leader; Adolf Hitler?
Can't tag this one 'humour' (and let's not forget the disastrous design of the Palm Beach butterfly ballot sheet in the 2000 US elections).
